Your experience and satisfaction with services are important. If you have a concern about services you received, you have several options available. You may choose to complete one or more of the steps below.
1) Request Your Health Care Records
You have the right to request a copy of your health care records directly from the licensee (provider). To request records, you may email me at DrLauraAustin@gmail.com to ask for instructions on obtaining your file. You may be asked to complete a written authorization form and/or pay a reasonable fee for copies, consistent with applicable laws and regulations.
2) Contact the Texas Behavioral Health Executive Council (BHEC)
